Removed sdf_subs.h from sdf_dll.zip since unpacking the zip file over-writes
the copy of sdf_subs.h that's already present, and didn't want to make
the zip file unpack things into a sub-directory.

New files to be included in the distribution are: sdf_dll.zip, containing
MinGW compiled versions of sdf.dll and sdf.def for use in Windows, and a
sample Makefile, Makefile_msvc for linking the sdf.dll into some example 
calling programs using the MSVC compiler.

Because of a comment from someone with only a low-bandwidth connection, the
file bz.sdf is no longer in the tarball, but will be available as a separate
file in the root directory of the SDF folder on solarmuri, ie
http://solarmuri.ssl.berkeley.edu/~fisher/public/software/SDF .

This file is needed if you compile and run the test fortran program 
test_sdf_transpose.F90 .
